Request/Update/Notification on Accreditation

Submit a request, update or notification about accreditation to the Exec Dir of Accreditation. Examples include: The dates of a site visit are set/changed; the accrediting body has changed standards/practices; you would like to schedule a meeting about getting started on your self-study; you are the new Accreditation Liaison for your department; your department wants to request an extended date on your next site visit; etc.
  • This form does not allow for document uploads. If you are submitting official documents from an Accrediting Body/Agency, use the "Official Accreditation Document(s)" form. If you are submitting a draft report for review, use the DRAFT document FOR REVIEW form.
